TITLE: Form G-1145: Request E-Notification of Application Acceptance

WHAT IT IS:

Form G-1145 lets you ask USCIS to send you a text message and/or email when they accept your application or petition. The message will include your receipt number and instructions on how to check your case status online. To protect your privacy, this notification will not contain your name or other personal details — text messages and emails are not secure enough for that.

WHO NEEDS IT:

You can file this form only if you are mailing one of the specific applications below to a USCIS Lockbox in:

- Lewisville, Texas

- Chicago, Illinois

- Elgin, Illinois

- Phoenix, Arizona

The eligible forms are:

- I-102 (Application for Replacement/Initial Nonimmigrant Arrival-Departure Document)

- I-130 (Petition for Alien Relative)

- I-131 (Application for Travel Documents, Parole Documents, and Arrival-Departure Records)

- I-140 (Immigrant Petition for Alien Worker)

- I-212 (Application for Permission to Reapply for Admission into the U.S. After Deportation or Removal)

- I-360 (Petition for Amerasian, Widow(er), or Special Immigrant)

If your form is not on this list or you are filing it at a different USCIS office, this e-notification service is not available for your case.

HOW TO FILE:

1. Download the form from the official USCIS website (see the link below).

2. Complete the form. Fill in your mobile number and/or email address where you want to receive the notification. The form will ask for your name, but remember: the notification USCIS sends will only show your receipt number, not your name.

3. Print the form. Check that the edition date (09/26/14) and page numbers are visible at the bottom of every page and that all pages come from the same form edition. If any page is missing or from a different edition, USCIS may reject the form.

4. Clip the completed Form G-1145 to the front of the main application or petition you are mailing. Use a paper clip so both forms stay together.

5. Mail the entire package to the correct Lockbox address for your main form. Form G-1145 does not change where you send your application.

COMMON MISTAKES:

- Using mismatched or missing pages: Every page must be from the same edition and the edition date must be visible at the bottom. If pages are mixed or cut off, your form may be rejected.

- Forgetting to clip the form to the front: If the G-1145 is not securely attached to the front of your main application, it might get separated and you will not receive a notification.

- Expecting a name in the notification: The text message or email will only contain your receipt number and case status instructions. Do not be alarmed that it does not include your name; that is for security.
