WHAT IT IS:

Form I-693 is the “Report of Immigration Medical Examination and Vaccination Record.” It is used when you apply for a Green Card (adjustment of status) to prove you are not inadmissible to the U.S. for health reasons. A USCIS-approved civil surgeon must fill out, sign, and give you this form.

WHO NEEDS IT:

You must submit Form I-693 if you are filing Form I-485 (Application to Register Permanent Residence). As of Dec. 2, 2024, you must include a completed or partial Form I-693 together with your I-485 application, or USCIS may reject your I-485.

Some applicants who already had a medical exam outside the U.S. may only need a partial form (Parts 1‑5, Part 7, and Part 10 – the Vaccination Record):

• Refugees (Form I-590 or I-730) who finished a panel physician exam abroad.

• Derivative asylees (Form I-730) who finished that exam abroad and apply for adjustment within 1 year of eligibility.

If the vaccination record from a panel physician exam abroad was not properly completed, these groups must also file that same partial form:

• K‑1/K‑2/K‑3/K‑4 nonimmigrants (spouse/child/fiancé(e) of a U.S. citizen) whose medical exam abroad was done no more than 1 year before filing for adjustment.

• Afghan nationals who arrived under Operation Allies Welcome and had a medical exam abroad.

Important: If your foreign medical exam found a Class A condition, or you may have developed one after arriving in the U.S., you might need a new full medical exam. Check the Form I-693 Instructions for what parts you must complete.

HOW TO FILE:

1. Find a USCIS-designated civil surgeon. The doctor examines you, completes the form, signs it, and seals it in an envelope.

2. If you file Form I-485 online (currently for certain employment-based applicants): open the sealed envelope, upload the completed Form I-693 with your online application. Keep the original form and envelope until USCIS makes a final decision—they may ask for them at an interview.

3. If you file Form I-485 by mail: include the sealed envelope from the civil surgeon, unopened, with your application package.

4. If USCIS rejects your application package and returns the I-693 envelope already opened, you may re-file a new package with that opened Form I-693 and a copy of the rejection notice.

5. Always send Form I-693 together with Form I-485—do not send it later.

CURRENT EDITION & DEADLINES:

Edition date: 01/20/25.

If the civil surgeon signs your form on or before July 2, 2025, you may use either the 03/09/23 edition or the 01/20/25 edition.

If the civil surgeon signs on July 3, 2025 or later, only the 01/20/25 edition will be accepted.

COMMON MISTAKES:

• Using an outdated form edition. Always confirm the edition date before your medical appointment.

• For a mailed I-485, breaking the sealed envelope—keep it closed unless you are filing online.

• When re-filing after a rejection, forgetting to include a copy of the USCIS rejection notice with the opened I-693.

• Losing the original I-693 and its envelope before USCIS issues a final decision (especially after online filing).
