WHAT IT IS:

Form I-134 is a promise, signed under penalty of perjury, that you will provide money support to a person who is coming to or staying in the United States for a temporary immigration benefit. It shows the U.S. government that the visitor will not become a public charge. You must file a separate form for each person you support.

WHO NEEDS IT:

Use this form if you are the financial sponsor (the U.S. supporter) for a beneficiary who needs a temporary immigration benefit. The beneficiary is the person you agree to support during their short stay. You file one Form I-134 for each beneficiary.
