WHAT IT IS:
Form N-565 is the application to get a new or corrected copy of a U.S. citizenship or naturalization document. You use it to replace a certificate that was lost, stolen, destroyed, or has a mistake, or to update it after a legal name change, date of birth change, or to reflect your biological sex at birth. You can also use it to request a special certificate of naturalization for use in a foreign country.
WHO NEEDS IT:
You should file this form if you already have a U.S. certificate of naturalization, certificate of citizenship, Declaration of Intention, or Repatriation Certificate, and you need a replacement because the original is:
- Lost, stolen, or destroyed,
- Contains a USCIS clerical or typographical error,
- Needs to be updated after a legal name change,
- Needs to be updated after a legal date of birth change,
- Needs to show your biological sex at birth, or
- You need a special certificate to present to a foreign government.
HOW TO FILE:
1. Decide how to file
You can file online or by paper mail.
2. Gather your supporting evidence
Use the following checklist (from USCIS) to know what to include. You do not send the checklist itself. If any document is in a language other than English, you must include a full English translation and a translator’s certification.
- Two identical passport-style photographs, if you live outside the United States.
- Your original certificate or document, if you are applying for a new or replacement document due to a name change, date of birth change, or to reflect your biological sex at birth.
- A copy of the original document, if you are replacing a lost, stolen, or destroyed certificate and you still have a copy.
- A police report or a sworn statement, if your document was lost, stolen, or destroyed.
- Evidence of a USCIS typographical or clerical error (if that is your reason for filing).
- Evidence of your legal name change, such as a marriage certificate or court order.
- Evidence of your legal date of birth change.
- Evidence of your biological sex (as required in the instructions).
3. Complete Form N-565
Use the current edition (see below). Make sure all pages are from the same form edition, and the edition date is visible at the bottom of every page.
4. File online (recommended for tracking)
- Create or log into your USCIS online account.
- Fill out and submit Form N-565 online.
- Pay the filing fee online.
- After you submit online, mail your original document to the Nebraska Service Center. The address is provided in the Evidence section of the online application. You must do this step.
5. File by mail
